67 year old man with acute myeloid leukæmia during his first cycle of remission-induction chemotherapy with idarubicin and cytarabine. He has a history of bicuspid aortic valve, hypertension and LVH, with normal left ventricular systolic function (ejection fraction 77%). He now developed febrile neutropenia and hypokalæmia (3.4 mEq/L).
