VT, then SVT
Report:
Sinus rhythm 90/min
PR 0.20”
Triplet of polymorphous ventricular tachycardia 120/min
5-beat run of atrial tachycardia 118/min
Left anterior hemiblock
Old anterior infarction
Comment:
Following the VT, an atrial escape beat is followed by the (sometimes overlooked) atrial tachycardia.
Note also, in the rhythm strip, the post-ectopic T wave change (poor man’s exercise test), more marked after the longer pause left by the VT than after the shorter one following SVT. In fact, the post-ectopic change is a function of the pause itself, rather than of a preceding ectopic beat.
