Long Wenckebach Pauses
Report:
Sinus rhythm 56 – 66/min
Second degree AV block, Möbitz 1
3:2 conduction with bigeminy
Left ventricular hypertrophy voltage
Comment:
Wenckebach pauses are normally less than two sinus cycles in length. When, as here, they coincide with the slow phase of respiratory sinus arrhythmia, they may be more than two sinus cycles long. There may be some ventriculophasic sinus arrhythmia as well.
The voltage of LVH may be a normal finding in a young adult. The only heart disease here is the (probably congenital) heart block. Like in the previous Case 111, it remains only second degree.
