Sinus Bradycardia: Escape-Capture Bigeminy
Report:
Sinus bradycardia 34 - 43/min
Junctional escape beats
Escape-capture bigeminy
Phasic aberrant conduction, RBBB type
Comment:
The mechanism of the bigeminy is sinus bradycardia. Here the effective sinus cycle is the same as the actual one; in other situations, AV block renders the effective cycle longer by multiples (depending on the block) of the sinus cycle.
The captures have slight variations in their “coupling” intervals. Note the reciprocal RP - PR relationship: the earliest P waves have the longest PR intervals – and aberrant conduction.
