Global T Wave Inversion
Report:
Sinus rhythm
Old inferior infarction
Probable left ventricular hypertrophy

Comment:
The patient was septic with peritonitis and had unstable blood pressure, with unexplained dips associated with relative sinus bradycardia. Nevertheless, no evidence of infarction or even ischæmia came to light. The striking T wave changes were gradually resolving as he left the ICU (Figs 123a, 123b).
Most cases of “spontaneous” global T wave inversion appear to be non-ischaemic, even though the subjects often have a background of coronary artery disease. The mechanism of this phenomenon remains unknown91.
