S1Q3T3 (McGinn-White) pattern
Anteroseptal T wave inversion consistent with right ventricular strain
Comment:
Tachycardia is not invariably present, especially with massive or submassive embolisation. This patient, like many others, had hypoxæmia and a clear CXR, with presenting acute dyspnœa and known predisposition to thromboembolism. She did not need an ECG to confirm the diagnosis, but it was nice to record the expected, classical pattern. We are all like Hobbits, preferring to confirm what we know already rather than be bothered with new things.
Below (Fig 165a) is a trace taken two days later, showing partial resolution of the pattern.
