Diffuse ST segment elevation suggestive of acute pericarditis

Comment:

The trace in very suggestive of acute pericarditis. The timid report of “inferior wall ischæmia” is preserved for students’ education. It became quite obvious later the same day (Fig 183a). This patient is unusual in that he developed episodes of atrial fibrillation (Fig 183b below). Atrial arrhythmias - contrary to the classical teaching - are rare in uncomplicated acute pericarditis133. They are over-represented in this collection precisely because of that.

The pain of acute pericarditis may be indistinguishable from that of acute myocardial infarction.

Echocardiogram showed a small but definite pericardial effusion. He was discharged home on steroids and warned there may be another recurrence.
