Comment:
The unusual occurrence of right-sided chest pain is well-recognised in patients with inferior infarction and dextrocardia148. The reason for this is not known.
It is best to take either a standard or a completely reversed set of leads in patients with dextrocardia. This example is one of the combinations only too often used by well-meaning staff.
The limb lead configuration of this patient is seen only in the mirror-image variety of dextrocardia. Isolated dextrocardia is not a cause of inverted P wave in lead 1.
