Comment:
This is surprisingly common, but the “incriminating” strips are rarely kept.
The VT run appears right ventricular in origin, given the general LBBB morphology. This indicates the CVC had just passed the tricuspid valve and should be withdrawn. It must be owned that, during a CVC (or Swan-Ganz catheter) insertion, a few VEBs are often welcome, indicating successful pass in the right direction. Nobody owns up to using it a guide, however.
