Comment:

Oddly enough the computer diagnosed “regular rhythm with unusual P axis”, as though negative P wave in L1 is merely “unusual”. It also went the full hog: “anterolateral infarct” and “right axis deviation”. The trace is diagnostic of mirror-image dextrocardia.

The precordial sequence is very characteristic: diminishing V1.

Important information from true chest leads (in this case, right-sided) may be missed if dextrocardia is unrecognised62. Below (Fig 91a) is this patient’s ECG with right-sided chest leads, revealing Wellens’ warning of proximal LAD stenosis.
