Report:

Upper strips:

Ventricular lead off

Atrial pacemaker rhythm 80/min

Loss of capture with reduced output

Escape sinus bradycardia

Resumption of capture at treshhold output

Lower strip:

Atrial pacemaker rhythm 53/min

Failure to pace in two cycles

Ventricular escape paced beats

Retrograde conduction

Comment:

Ventricular lead is turned off in the upper strips. The atrial spike size indicates this is a treshold exercise. The output should have been left higher judging from loss of capture in the lower strip, taken a few minutes later.

Failure of atrial activation there creates a longer pause and allows the ventricular-paced beat to occur. It also makes retrograde conduction more likely. The next atrial spike is not delayed, not being subject to post-ectopic depression.
